The lessons learned concept has been around for many decades and has been used in multiple fields to improve future outcomes and avoid repeating errors. This concept allows one to systematically revisit a past experience or incident and understand how certain changes could have influenced a better outcome. In keeping with the theme of the conference “Forensic Science & Public Safety: A Caribbean Journey” we need to ask why isn’t that concept of lessons learned widely accepted or practiced in the Caribbean. This concept is not new, and in the Caribbean we do not need to reinvent the wheel to adopt or put it into use. It simply involves learning or improving from our mistakes and past experiences of incidents, with the end goal of achieving a more favorable outcome.

This presentation will touch on multiple disciplines or aspects of public safety in the Caribbean region, that have been ignored and if circumstances were to repeat themselves would result in the same or greater tragedy.
